When moving Mother and Baby from one ward to another within the same hospital you will need to complete the transfer and tracer of the hospital notes on PAS. If you transfer via Swiftplus you will still need to tracer the notes on PAS. If the baby goes to Neonates from Delivery suite, there is no need to complete an E3 transfer as the birth questionnaire will ask where the baby was transferred to, so will capture those babies transferred to N N U.
Transferring Mother and Baby into the community is done via the postnatal care folder.
The Postnatal Transfer to community questionnaire needs to be completed. Once the questionnaire has been completed, a pop up box will appear reminding you if baby is being transferred the baby transfer questionnaire needs to be completed within the baby record. Baby’s record can be accessed via the baby icon in the icons within the mother’s record.
Once the questionnaire has been completed a print box will pop up. Print the transfer documentation.
You will need to complete the Mother Postnatal Transfer to Community and Babys Transfer to community documentation separately, as you complete each transfer form, the system will automatically send a notification to the GP and community team. Remember Mother and Baby must be discharged on PAS.
