Discharge from Care

The final discharge from care questionnaire is completed by Community Midwives when midwifery care is completed.

Before the questionnaire is completed it will be called ‘Final Discharge’, once you enter the discharge details it will show as Discharged from Care with the date and time completed.

Once the questionnaire has been completed a print box will pop up. Print the discharge documentation.

There is no discharge on PAS at this time, as the patient would have been discharged already on PAS on transfer into the community; however this data is really important for our unit figures to record breast feeding rates on transfer to the Health Visitor for example.

Remember to also complete the final discharge on the baby record.
